New Patients
Whether you are a new patient, experiencing dental pain, or coming in for a routine cleaning and exam here is what you will need to prepare for your visit!
Before your visit:
For new patients we ask you to complete new patient paperwork. This includes a dental history, release of information, financial policy, informed consent, medical history and patient financial agreement. Should you have any questions regarding your insurance plan, Shawn will assist you in how to best utilize your benefits in order to complete any recommended treatment.
Patient Experience:
Upon your arrival, you will be greeted by Shawn who will check you in and ensure all questions are answered. We ask that all paperwork be completed prior to your first visit. If you are not able to complete it, we ask that you please show up at least 15 minutes early to do so.
Once we are ready for you, one of our friendly team members will assist you back to your treatment room and review medical history and any concerns you may have. This will be followed by any recommended radiographs (X-rays) so that Dr. Kashou may accurately assess the conditions of your oral health. He will perform a thorough oral exam, take intra oral photos if necessary, and check for issues such as tooth decay, gum disease, and other problems that can affect not only your mouth but your overall health.
Prior to dismissal, you will receive a detailed print out of your personalized treatment plan and Shawn will review any patient financial responsibilities you may have. Dr. Kashou treats all patients as he would his own family, only recommending what is necessary. He looks forward to meeting you and assisting you in achieving optimal oral health!

Insurance Information
Avocado Family Dentistry accepts most major insurance plans where you choose your own dentist. We are currently contracted with the following:
- Delta PPO and Premier
- Aetna PPO
- Guardian through Connection Dental, Zelis, Careington Care PPO
We accept all major credit cards as well as Care Credit and flex plans. We also provide an exciting new patient special for patients without insurance. The plan includes full mouth x-rays, 2 exams, and a standard cleaning. For more details regarding cost, non-insured retiree cleanings, and our exam and X-ray special please call our office.
